Call 972-529-6939 and press 2 for appointments.
Step 2
We utilize an electronic check in process. You should receive an email with a link to complete the online forms approximately 48 hours prior to your appointment. If you do not receive the email or do not have a chance to complete the pre-registration online please arrive at least 20 minutes early to complete the registration before your appointment start time.
Step 3
Please bring the following items with you to your appointment:
- Health Insurance card
- Photo ID
- Copies of any recent labs or other diagnostic test results.
- Primary Care Physician Referral (if required by your insurance plan). Please make sure you obtain this before you appointment.
- Payment method. For co-payments we accept personal checks, Visa, MasterCard, Discover, American Express, Care Credit and cash.
If you do not have the ability to complete your online check in, please arrive 20 minutes prior to your appointment time to complete it in our office.
